Today, I got this email from OpenAI:

For the record, I have no idea what "Cyber Abuse" is. I pay $200 a month for ChatGPT Pro, so I would prefer not to have my account deactivated.

Each option resolves YES if my OpenAI account is deleted or deactivated by OpenAI without my consent before its listed month/year (in Pacific Time), and NO otherwise.
